If you suffer from persistent constipation, bloating, trapped wind or abdominal distension, you may have come across the term “methane SIBO”.
Methane SIBO is particularly associated with constipation and slower movement through the digestive tract. However, not everyone with elevated methane experiences exactly the same symptoms, and you do not necessarily have to be severely constipated to have it.
In fact, “methane SIBO” is not technically the most accurate name for the condition. It is now more commonly referred to as intestinal methanogen overgrowth (IMO).
In this article, we explain the most common methane SIBO symptoms, why methane is associated with constipation, how methane SIBO differs from hydrogen SIBO, and how IMO can be treated and tested.
What are the symptoms of methane SIBO?
The most commonly reported symptoms associated with methane SIBO, or intestinal methanogen overgrowth, include:
- constipation
- bloating
- abdominal distension
- excessive gas or trapped wind
- abdominal pain or discomfort
- straining to pass stools
- a feeling of incomplete bowel evacuation
- nausea
- changes in bowel frequency or stool consistency
Constipation is the bowel symptom most strongly associated with methane, but bloating, abdominal pain and flatulence are also frequently reported.
A 2025 systematic review and meta-analysis looking at patients with intestinal methanogen overgrowth found that approximately:
- 78% experienced bloating
- 65% experienced abdominal pain
- 56% experienced flatulence
- 51% experienced constipation
- 30% experienced nausea
- 33% experienced diarrhoea
What is methane SIBO?
“Methane SIBO” is the term most people still use, but the more scientifically accurate name is intestinal methanogen overgrowth, or IMO.
Why?
SIBO stands for small intestinal bacterial overgrowth, but methane is not actually produced by bacteria.
It is produced by microorganisms known as methanogens, which belong to a different group of organisms called archaea.
Methanogens can also live in both the small and large intestine, which means describing the condition purely as a small intestinal bacterial overgrowth is not technically correct.
For these reasons, the term intestinal methanogen overgrowth (IMO) is more accurate.
Is constipation the main symptom of methane SIBO?
Constipation is the symptom most consistently associated with elevated methane levels.
However, constipation is not present in everyone with IMO.
Why does methane SIBO cause constipation?
Methane is linked with constipation mainly because it appears to slow the movement of the intestines. Research has consistently found that people who produce more methane tend to have slower intestinal transit, and higher methane levels are associated with more severe constipation.
The exact mechanism is still being studied, but experimental research suggests methane can change the way the intestinal muscles contract. Instead of the coordinated contractions that move food and waste forward efficiently, methane appears to increase contractions that do not propel contents forward as effectively. The result can be slower transit, harder stools and more difficulty emptying the bowel.
What causes methane SIBO?
There is not usually one single cause. Methane SIBO, or IMO, is often linked with a slow-moving gut. When food and waste stay in the intestines for longer, this may create conditions that allow methane-producing organisms to thrive. Methane itself may then slow gut movement further, potentially creating a cycle of slow transit, more methane and constipation. Other factors that affect gut movement, including certain medications, IBS-C and previous abdominal surgery, may also play a role.
Can you have methane SIBO without constipation?
Yes. You can have IMO without constipation.
Although constipation is strongly associated with methane, it is not present in every methane-positive patient.
A 2025 systematic review found constipation in approximately 51% of people with IMO whereas 33% had diarrhoea.
This means that constipation is associated with IMO, but it is not required for someone to have the condition.
It is also important to remember that bowel frequency alone does not define constipation.
Someone may open their bowels every day but still experience hard stools, straining or a feeling of incomplete evacuation.
Does methane SIBO cause bloating?
Bloating is very common in IMO — a 2025 systematic review found it in around 78% of people with intestinal methanogen overgrowth. Bloating is one of the most commonly reported symptoms of IMO.
Methane SIBO, or IMO, may contribute to bloating because methane-producing organisms are involved in the fermentation of food in the gut. During this process, gut microbes produce gases such as hydrogen, which methanogens then use to produce methane. This build-up of gas can contribute to feelings of fullness, pressure and bloating. Can methane SIBO cause abdominal distension?
Yes. Abdominal distension can occur alongside methane SIBO or IMO.
Some people report that their abdomen appears relatively flat in the morning but becomes increasingly distended following meals or as the day progresses.
Constipation, altered gut motility and gas production may all contribute.
Does methane SIBO cause trapped wind and gas?
Gas and flatulence are common symptoms in people with IMO.
Methanogens have an interesting relationship with other microorganisms in the gut.
During fermentation, certain bacteria produce hydrogen.
Methanogens use some of this hydrogen and convert it into methane.
This is one reason why a SIBO breath test should ideally measure both hydrogen and methane rather than looking at methane alone.
What does methane SIBO feel like?
There is no single way that methane SIBO feels.
However many patients describe
constipation + bloating + abdominal pressure + trapped gas and brain fog as well as feeling very full after eating.
Some patients describe feeling as though their digestion is extremely slow.
They may struggle to pass stools, feel that their bowel has not completely emptied, and experience increasing abdominal pressure or distension throughout the day.
Others may have much less constipation but experience pronounced bloating, abdominal discomfort or gas.
Can methane SIBO cause diarrhoea?
Yes, some people with IMO experience diarrhoea, although methane is much more strongly associated with constipation.
The 2025 systematic review found that around 33% of people with IMO reported diarrhoea.
Some people also experience mixed bowel habits, alternating between harder and looser stools.
Can methane SIBO cause nausea?
Yes, nausea can occur in patients with intestinal methanogen overgrowth.
In the 2025 systematic review, nausea was reported by approximately 30% of people with IMO.
Some people describe feeling:
- unusually full after eating
- mildly nauseated after meals
- as though food is taking a long time to move through the digestive system
Can methane SIBO cause weight gain?
Some studies have found that people who produce more methane on breath testing tend to have a higher BMI or more body fat, suggesting there may be a link between methane-producing organisms and weight regulation.
However, these studies show an association, not cause and effect. Weight is influenced by many factors, and current evidence is not strong enough to describe weight gain as a typical or diagnostic symptom of intestinal methanogen overgrowth. A recent review also recommends a cautious approach to claims about IMO beyond its clearer association with constipation and altered gut motility.
Does methane SIBO cause fatigue or brain fog?
Many patients report fatigue, difficulty concentrating or “brain fog”.
These symptoms are also frequently discussed online in relation to SIBO.
Fatigue and brain fog can have many other causes, including poor sleep, stress, iron or vitamin B12 deficiency, thyroid problems and other medical conditions. For this reason, they are better described as possible associated symptoms rather than typical or diagnostic signs of methane SIBO.
What is the difference between methane SIBO and hydrogen SIBO?
Methane and hydrogen breath-test patterns tend to be associated with somewhat different bowel symptoms.
Methane SIBO / IMO
More commonly associated with:
- constipation
- slow bowel movements
- bloating
- abdominal distension
- trapped gas
- incomplete evacuation
Hydrogen SIBO
More commonly associated with:
- diarrhoea
- loose stools
- bloating
- abdominal pain
- gas
However, there is considerable overlap.
People may also produce both hydrogen and methane, so the distinction is not always straightforward.
This is another reason it is useful for breath testing to measure both gases.
Is methane SIBO the same as IBS-C?
No. IBS-C and intestinal methanogen overgrowth are not the same condition.
IBS-C means irritable bowel syndrome with constipation.
IMO refers to excessive methane production associated with intestinal methanogens, usually identified through breath testing.
However, there is considerable overlap between the two.
Research has found methane-positive breath tests to be more common in people with IBS-C than in people with diarrhoea-predominant IBS.
This means IMO may be worth considering in someone who has persistent IBS-C symptoms, particularly when constipation occurs alongside pronounced:
- bloating
- abdominal distension
- trapped gas
- abdominal discomfort
However, not everyone with IBS-C has IMO.
How do you know if you have methane SIBO?
You cannot reliably diagnose methane SIBO from symptoms alone.
The symptoms overlap considerably with other digestive problems including:
- IBS
- functional constipation
- pelvic-floor dysfunction
- coeliac disease
- food intolerances
- other gastrointestinal disorders
A hydrogen and methane breath test is commonly used when intestinal methanogen overgrowth is suspected.
The test usually involves drinking either glucose or lactulose and then providing breath samples over a period of time.
These breath samples measure the amount of hydrogen and methane being produced within the digestive tract.
What methane level is considered positive?
Current North American breath-testing consensus recommendations consider a methane level of:
10 parts per million (ppm) or greater at any point during the breath test
to be methane-positive.
Is methane SIBO the same as intestinal methanogen overgrowth?
Essentially, “methane SIBO” is the commonly used term for what is now more accurately called intestinal methanogen overgrowth (IMO).
IMO is the preferred terminology because:
- methane is produced by archaea rather than bacteria
- methane-producing organisms may be found in both the small and large intestine
So if your breath-test report refers to IMO, it is describing the condition commonly called methane SIBO.
Can methane SIBO come back?
Methane SIBO, or IMO, can return after treatment. This may be more likely if the underlying problem has not been addressed, particularly slow gut motility or ongoing constipation.
If food and waste continue to move slowly through the intestines, it can create conditions that allow methane-producing organisms to build up again. This is why treatment often needs to look beyond simply reducing methane and also consider why the gut is moving slowly in the first place.
Factors such as certain medications, previous abdominal surgery and conditions that affect gut motility may also contribute.
Potential factors may include:
- slow intestinal transit
- certain medications
- previous gastrointestinal surgery
- underlying digestive disorders
- conditions affecting intestinal motility
The underlying factors differ considerably between individuals.
How do you treat Methane SIBO?
Treatment for methane SIBO, or IMO, usually aims to reduce methane-producing organisms and improve gut motility.
Prescription antibiotics such as rifaximin, sometimes combined with neomycin, may be used under medical supervision. Some practitioners also use herbal antimicrobials, although the evidence for these is less established.
It is also important to address constipation and slow gut movement, as these may contribute to the problem returning.
Dietary changes can help manage symptoms such as bloating and gas, but diet alone does not necessarily treat IMO.
